According to Healthy People 2020, heart disease is the leading cause of death in the United States and stroke is the fifth leading cause. Heart disease and stroke, along with other cardiovascular disease, are the most widespread and costly health problems in our country. However, they are also considered the most preventable. For this clinical experience, I desire to help improve cardiovascular health and quality of life through prevention, detection, and treatment of an individual’s risk factors for cardiovascular disease. The aggregate of focus will be males with a family history of hypertension living in Appling county between the ages of 20-30 who meet the criteria for prehypertension. I will begin by first assessing what the patients’ individual risk factors are. Next, I will provide education on the importance of implementing a healthy lifestyle in order to help prevent cardiovascular problems. I will stress that there should be more focus on prevention as opposed to waiting for disease to develop before focusing on such risk factors. Then, with the patient, we will create an individualized plan to incorporate a healthier lifestyle based on factors such as the client’s resources and readiness to change. For evaluation, the patient will come in weekly to discuss how, if any, changes were made and what effects do they believe it has had on their health. I will also use their blood pressure trend with each visit as an indicator if any positive change has been made. Concepts of Community Health Nursing
