Therefore, it’s important to have a firm understanding of the types of instructional notations that you’ll find in ICD-10. Test your knowledge of several important ICD-10 coding con- ventions by answering each of the following questions:

Part 3: Assign Multiple Codes

Review each of the following diagnoses. Assign the ICD-10 diagnosis codes and procedure codes, according to the instructions.

1. A patient is diagnosed with an emphysematous bleb, duodenitis without bleeding, and primary spontaneous pneumothorax. Assign the ICD-10-CM codes.

2. A patient undergoes an excision of a lesion from the left thyroid (open approach), and an excision of a lesion from the upper gum tissue (open approach). The physician also inserts an anterior nasal packing into the left nos- tril to control nose bleed (that is, epistaxis). Assign the ICD-10-PCS procedure codes.

3. A patient is diagnosed with a concussion with loss of consciousness of 16 hours after falling from a window at an office building. The physician performs a right occipi- tal craniectomy to treat the brain injury. Assign the ICD-10-CM code(s) and ICD-10-PCS procedure code.

Part 3 is worth 55 points of your overall graded project.

GOAL Your goal is to demonstrate your understanding of ICD-10 and to accurately assign ICD-10 codes.

GRADING CRITERIA The graded project will be evaluated according to the following criteria:

Content For Part 1, the student

� Writes at least one to three paragraphs in each of the three columns for the table

� Writes in complete sentences, not just simple yes or no statements

� Highlights at least three key concepts in each of the three columns

� Stays focused on the assigned issues

� Writes in his or her own words with correct grammar, spelling, and punctuation

For Parts 2 and 3, the student

� Includes all required characters in each code

� Includes required decimal points in correct positions within each code
